I know this is an old thread but just in case anybody else is having
this problem. Regarding question one, check your userapps log for this
error "LDAP: error code 4 - Sizelimit Exceeded" and check if the number
of groups in your scope (container) exceeds 200, apparently there is a
bug when setting page permissions for groups where if the result
returned exceeds 200 you will get the "PortletContainerImpl: failed to
run the portlet: 'PortalPagePermissions'. Please contact your System

This however does not seem to happen with users or containers where you
will get a very nice message "(Your query has returned more than the
maximum of 200 results. The first {1} results are shown below. Try a
more specific search criteria.):"

So in order to solve the issue, either use a more specific search
criteria (which returns less than 200 results) or change the maximum
size limit as stated here
